Circuit Surgeon โ Repair Service Agreement
1. Diagnostic & Repair Authorization
By submitting this form, you authorize Circuit Surgeon to perform a diagnostic evaluation of the described device. No repair work beyond initial diagnostics will be performed without your explicit approval of the estimated cost. You will be contacted with findings and a quote before any billable work begins.
2. Limitation of Liability
Electronics repair involves inherent risk. Circuit Surgeon will exercise reasonable professional care in handling your device; however, we cannot be held liable for pre-existing conditions, latent defects, or failures that occur during or after service that are unrelated to the work performed. Devices that are damaged, corroded, liquid-affected, or otherwise compromised may carry additional risk of failure during repair โ this will be disclosed during diagnostics. By submitting your device for service, you acknowledge and accept these risks.
3. Data & Storage Media
Circuit Surgeon is not responsible for any data loss that occurs during repair. It is your responsibility to back up all data before submitting a device for service. We recommend backing up all storage media prior to service regardless of the nature of the repair.
4. Abandoned Property Policy
Devices not claimed within 30 days of notification that service is complete (or that a quote is ready and awaiting approval) will be considered abandoned property. Circuit Surgeon reserves the right to dispose of, sell, or otherwise recoup costs associated with abandoned devices after this 30-day period. We will make at least two reasonable attempts to contact you via phone and email before exercising this right. Prepaid repair fees are non-refundable on abandoned devices.
5. Payment
Payment is due upon completion of service and before the device is released. Accepted payment methods will be confirmed at the time of service. Devices will not be released until payment is received in full.
6. Warranty
Repair work is warranted against defects in our labor for 30 days from the date of service completion. This warranty covers only the specific repair performed and does not cover new or unrelated failures, physical damage, liquid damage, or software issues occurring after pickup. Parts carry manufacturer warranty where applicable.
7. Right to Refuse Service
Circuit Surgeon reserves the right to decline any repair request at our discretion, including but not limited to devices that pose safety concerns or repairs outside our current service capabilities.
